Does anyone use a different intake form for kids being referred for language-literacy evaluations rather than standard speech-language evals? If so, would you be willing to share?
Thanks in advance,
Leslie

Linda Lombardino responded < I have two in my book. One for younger and one for older kids. Due out right before ASHA. >
Note: Linda is doing the Lexercise Struggling Readers Live Broadcast on November 2nd so you could talk to her then about her new book and her intake forms, etc.

Sandie,
I am interested in this live broadcast and am on the email list to receive info about webinars, but until now haven't heard about this upcoming event, nor can I find the link to register or info about it from the link you sent. Hmmmm . . .
It hasn't gone out yet.... and won't until after today's Live Broadcast. (We just send out one invitation one at a time to keep from confusing people.)
But you can always read the upcoming schedule on the Lexercise Blog to see what is coming up. If Linda's Broadcast interests you I suggest you save the date/time on November 2nd and look for our registration invite in the next few weeks.
